급성신부전 (急性腎不全)에서의 지속적 (持續的) 복막투석 (腹膜透析)

Abstract
Twenty four patients with acute renal failure- who have been hospitalized and were treated by continuous peritoneal dialysis at Keimyung Un- iversity Dongsan Medical Center from June 1984, to February 1985. The following results were obtained. Two third of all cases were seen between third and fifth decade and sex ratio was 1.4 to 1 of male preponderance. Epidemic hemorrhagic fever- was a most common cause of acute renal failure, which account for two third of all cases. The re- maining six patients had diverse etiologies. The major indications for continuous peritoneal dialy- sis were: active bleeding in 9(37, 5% ), noncata- bolic state in 8(33. 3%), poor vascular access in 4 (16.7% ), and unstable cardiopulmonary status in 3(12. 5%) patients. The mean duration on contin-- uous peritoneal dialysis was 9 days and the mean daily volume of ultrafiltration was 1,607 ml. The BUN and serum creatinine, K, phosphorus, uric acid concentration decreased significantly, whereas serum sodium, CO, content increased significantly after continuous peritoneal dialysis. But the ser- um calcium, blood glucose, and the serum albumin did not change significantly by continun periton-- eal dialysis The major complications of continuous peritoneal dialysis were hypakalemia 66. 7%, ple- ural effusion 25%, pulmonary edema 12. 5 % and peritonitis, blood oozing at incision site, hyper-- glycemia, platelike atelectasis 8. 3 % respectively. In our result overall mortality rate was 8.3. Percentage and only two of twenty four patients with acute renal failure died of progression of underlying condition. In summary, continuous peritoneal dialysi s appears to be a effective and advantageous ther apy for most patients with acute renal failure, except for those with prolonged anuria. Technical complications are rarely a cause of serious mobi- dity. Extraperitoneal complications are acceptable and easily m anageable. The overall prognosis is good and suggests a significant improvement.
