성인 미세변화형 신증후군에서 서택적 단백뇨지수의 의의

Abstract
One hundred one adult patients with biopsy-proven minimal change nephrotic syndrome (MCNS) were investigated during the period of 5 years from Jan. 1981 to Dec. 1986 to evaluate the significance of selective proteinuria index (SPI) in adult minimal change ne- phrotic syndrome. The results obtained in this study were as follows: 1) Among 101 MCNS patients, 69 were males and 32 were females. The male to female ratio was 2.1 to 1 and their mean age was 29.7 years. 2) 58 out of 82 patients (71%) showed complete remis- sion with steroid treatment. 3) Of 49 cases in whom SPI measurements were car- ried out, 15 cases (31%) presented with slective proteinuria showing values below 0.15. 4) Complete remission rate with steroid therapy was 79% in patients with selective proteinuria, which was significantly higher than that of non-selective proteinur- ia group who showed only 58% of remission rate. 5) Among the patients in whom complete remission was achieved with steroid therapy, non-relapse group showed a significantly lower SPI values than frequent relapse group. 6) No significant differences were noted in the labora- tory findings or the frequency of complication between selective and non-selective proteinuria group. In conclusion, even in adult MCNS, SPI could be empolyed as an useful indicator to predict steroid re- sponse as well as the pattern of relpase.
