CAPD 중 발생한 급성충수돌기염과 분변성 복막염

Abstract
A 41 year-old man on CAPD had a severe peritonitis due to perforated appendicitis. The main clinical fea- tures included fever, cloudy peritoneal effluent, diffuse abdominal pain and rebound tenderness and leu- kocytosis with a shift to the left. Culture of the effluent grew Escherichia coli. The peritonitis did not respond to antibiotic therapy and removal of CAPD catheter. The diagnosis was rnade 10 days after admission by an exploratory laparatomy. The patient died 18 days after the operation. Our experience suggests that a perforated appen- dicities should be suspected in a patient with severe symptoms, leukocytosis with a shift to the left and E. coli growing in dialysate effluent, particularly in a patient without diverticular diseases.
