낭창성 신염의 임상 및 병리학적 고찰

Abstract
We investigated the clinical manifestations, labora- tory findings and renal biopsy findings of 100 patients diagnosed as lupus nephritis under admission to Yonsei Medical Center from Jan., 1979 to Apr., 1988, and the following results were obteined. 1) The mean age on admission was 28.6±10.4 years and male to female ratio was 1:6.1. The majority of patients belonged to the third and fourth decades of life. 2) On admission, 26 cases showed decreased renal function with serum creatinine more than 1.5 mg/dl, 27 cases proteinuria more than 3.5 gm/day, and 5 cases isolated hematuria. 3) The incidences of ARA criteria for SLE revealed that the antinuclear antibody (ANA) was 96.9%, hematologic disorder 88%, immunologic disorder 75%, malar rash 56%, arthritis or arthralgia 50%, discoid rash 44%, serositis including pleuritis and pericarditis 39 oral ulcer 17%, neurologic disorder 17% and photosensitivity 16%. 4) The most common clinical feature was fever with 77%. Pulmonary disorder was 47%, hypertension 36%, cardiologic disorder 35%, hepatomegaly 30%, gastro- intestinal disorder 25%, Raynaud phenomenon 20%, lymphadenopathy 19%, purpura 17% and splenomegaly 15%. 5) In the immunologic laboratory findings, positive rate of the ANA was 96.9%, anti-DNA antibody 68.6%, direct Coombstest 65.1%, LE cell preparation 57.6%, rheumatoid factor 30.7%. The incidence of decreased C3 titer was 71.6% and C4 titer 80.9%. The increased of gammaglobulin was 61.6% and reversed A/G ratio 53.5 6) On admisson, the most common hematologic abnormality was anemia 90%. Lymphopenia was 73%, leukopenia 48%, thrombocytopenia 43% and pan- cytopenia 22%. 7) In 41 cases, renal biopsy was performed and the most common histologic type was diffuse proliferative nephritis.
