성인에서 신증후군으로 발현된 원발성 막성신병증의 임상상

Abstract
Membranous nephropathy is the glomerular disease characterized by diffuse thickening of the glomerular basement membrane without proliferative change and is usually manifested by the nephrotic syndrome. To eval- uate clinical characteristics of this disease in adults, we analyzed 57 Patients of idiopathic membranous ne- phropathy with nephrotic syndrome who were diagnosed at the Department of Internal Medicine, Seoul National University Hospital from January 1979 to December 1986. The results are as follows; 1) Idiopathic membranous nephropathy was 16.8% of total adult idiopathic nephrotic syndrome and occurred most frequently at the sixth decade. Average age was 38.0 and sex ratio was 1.9:1. 2) Edema was observed in 85.5% of cases. Hyperten- sion and oliguria was observed in 2S.6% and 3.6% respectively. 3) Microscopic hematuria was detected in 38.6% and azotemia was found in 17.5% of cases. Selectivity index was above 0.2 in 68.8% of cases. There was no statistical correlation among hypertension, azotemia and hematur- ia. 4) Interstitial change was noted in 15.8% of cases and showed statistically significant relation to azotemia. Pathologic stage showed tendency to progress according to the duration of renal disease. 5) Among 31 patients who were followed more than 25 weeks after steroid treatment, 12 cases showed clini- cal response. There were no clinical indices that distin- guished response group from no-response group except pathologic stage.
