만성신부전에서 골변화의 정량적 측정

Abstract
The renal osteodystrophy (ROD) is one of important complications in chronic renal failure. Therefore, the quantitative assessment of bone mineral density is essential for effective treatment and prevention of ROD. However, the fact that several different methods are being used concurently indicates that no simple and reliable method has yet firmly established. Consequent- ly, the assessment of the degree of bone density hased on the metacarpal index and using on aluminium-step wedge, microdensitometer and computer were proposed by Inoue. We evaluate the bone changes of 37 CRF patients (19 women and 18 men, age range 17-71 year, creatinine clearence 10.1±5.7ml/min) with MD method included scoring system. The obtained data were compared with those in normal Korean population and the results were as fol- lows. 1) An overall bone mineral content (BMC) increases up to the age 30 years, where as decreases markedly with increase after the age of 30 years in CRF (p<0.001). 2) The sum of total count of each bone index was high significantly in CRF rather than normal Korean population (p<0.05). 3) Gsmax was higher in CRF than normal Korean population (p<0.001), other indices also were high in CRF (MCI, d, ? Gsmax, ? GS/D). 4) BMC of CRF patients who were undergone hemodialysis was no significant interval changes after 3 months. In conclusion, microdensitometry with a scoring sys- tem is very useful method to evaluate ROD in CRF and BMC is rapidly decreased after age 30 year in CRF patients.
