Abstract
The relation of thirst and interdialytic weight gain to the renin-angiotensin system was studied in 30 patients with chronic renal failure receiving regular hemodi- alysis treatment. They were divided into two groups, ie, group A (15 patients), interdialytic weight gain below 0.5 kg/day with absence of thirst; group B (15 patients), inter- dialytic wegith gain above 1kg/day with excessive thrist. The plasma renin activity, daily salt intake and ratio of interdialytic weight gain to daily sodium gain were significantly increased in group B. There was significant positive correlation between interdialytic weight gain and post-dialysis plasma renin activity with the correlation coefficient of 0.628. Interdialytic weight gain and thirst were significantly reduced after administration of an angiotensin convert- ing enzyme inhibitor, enalapril. The result suggests that excessive thirst and subse- quent interdialytic wight gain in some regular hemo- dialysis patients seem to be mediated by increased endogenous angiotensin II.
