혈액투석에서 쇄골하정맥 이중도관 사용에 대한 임상적 연구

Abstract
Subclavian dual lumen catheter(SDLC) has gained wide acceptance for short-term hemodialysis with the realization of the need to spare peripheral extremity vessels for the creation of more durable internal arter- iovenous fistulas. During 18 month-period between May 1986 and Oct. 1987, seventy-one SDLC were inserted into 64 patients and 359 hemodialyses were performed: 1) Catheters were retained in place for an average period of 28.7 days (5-64 days) and the average dialyses was 5.1 sessions/catheter. 2) The main reasons for SDLC insertion were previ- ously undiagnosed ESRD without vascular access, acute renal failure and urgent transfer from CAPD failure. 3) Complications related cannulation were transient local bleeding and puncture of subelavian artery in approch process, but serious traumatic complications such as pneumothorax or hemopericardium did not occurred. 4) Major dialysis-specific problems were poor blood flow and malfunctioning in one of double lumens, and such mechanical problems were more frequent on left subclavian cannulation than right side insertion. 5} There was no significant difference in preservation of catheter patency between daily priming with low- concentrated heprin and interdialytic priming with high- concentrated heparin.
