Abstract |
To investigate the role of type I hypersensitivity on the pathogenesis of idiopathic nephrotic syndrome, we evaluated the incidence of allergy history, and al]ergen skin test, allergic specific IgE (RAST), basophil his- tamine releasability test were performed in nephrotic syndrome patients. The study included 28 idiopathic (MCNS 12, FSGS 8, Steroid Responsive NS 8) and 8 secondary (HBV as- sociated MN 4, PSGN 2, SLE 2) nephrotic patients. The results were as follows: 1) There was no statistically significant difference in the incidence of allergy history between the idiopathic N.S. (28%) and secondary N.S. (13%) (p = 0.44) 2) The % of patients number with positive results of skin test, RAST, basophil histamine releasability test vere 14%, 11%, and 11%. Respectively in idiopathic N.S. and the secondary N.S. did not show any positive result. But also there vas no significant statistical difference between two goups. 3) The serum level of total IgE in the patients with idiopathic N,.S. (704± 1340 IU/m]) was elevated, compar- ed with the patients with secondary N.S. (110± 566 IU/ ml) (p=0.03). However, there was no statistically signifi- cant difference between the allergy history positive patients (1138±1981 IU/ml) and allergy history negative patients (530±1001 IU/ml) (p=0.43) among idiopathic N.S. In conclusion, the observed significant higher level of IgE in idiopathic nephrotic syndrome patients could not be explained by the significant role of type I hypersen- sitivity on the pathogenesis of idipathic nephrotic syn- drome. |
