만성사구체신염 환자에서의 사구체내 Fibrinogen 혹은 Fibrin 침착에 관한 연구

Abstract
Recent studies suggest that intraglomerular coagula- tion plays a role in experimental glomerulonephritis. The purpose of this study is to see whether the fibrin(ogen) deposition in glomerulus is different between various kinds of glomerular pathologic findings and to see whether the fibrinolytic activity in plasma effects to the deposition of it. We performed the retro- spective review of renal biopsy and measured plasma fibrinolytic activity. The results were as follows; 1) Glomerular deposition of fibrinoid was not obser- ved in patients with minimal change nephritis. 2) Glomerular deposition of fibrinoid was frequently observed in patients with membranous glomerulone- phritis(2 of 5 patients, 40 g), membrano-proliferative glomerulonephritis(2 of 6 patients, 33%), glomeruloscle- rosis(1 of 4 patients, 25%), IgA nephritis(4 of 13 patients, 31%), Henoch-Schonlein purpura(1 of 2 patients). 3) Euglobulin fibrinolytic activity and t-PA, serum- albumin and 24 hour urine albumin in patients with glomerular fibrinoid deposition were not different sig- nificantly form the patients without it. 4) Patients with fibrinoid deposition showed the higher incidence of glomerular C3 deposition than the patients without it(p<0.001). 5) Patients with fibrinoid deposition showed the higher incidence of glomerular deposition of electron dense deposits than the patients without it. In conclusion, the incidence of fibrin(ogen) deposit was higher in membranous GN, membranoproliferative GN, IgA nephritis than minimal change nephritis. The finding of concurrent deposit of C3 and electron dense deposit with fibrin(ogen) deposit suggests that fibrin(ogen) deposition is linked to the complement activation and immunologic tissure injury.
