경피적 혈관확장술에 의한 중심정맥협착의 치료

Abstract
Percutaneous transluminal angioplasty (PTA) has played an important role in the management of malfun- ctioning fistula. Stenotic lesions, most commonly occur- ring in the venous limbs of such fistulas, are the most common anatomic cause of malfuction. Central vein stenosis has emerged as a serious new medical complica- tion of hemodialysis catheter insertion. These central vein lesions are not amenable to simple surgical revision or thrombectomy because of their intra-thoracic loca- tion. Previous reports have documented the excellent response of subtotal central vein lesions to PTA, howe- ver, there was no report in Korea. We performed PTA succesfully in 1 patient presenting with near-total obstruction of the subclavian vein after subclavian catheterization and arterio-venous fistula at the same side. The pressure difference across the stenotic site was 30 mmHg, which decreased to 0 mmHg after PTA. Arm swelling and collateral circulation also decreased remarkably after PTA.
