남성 낭창성 신염의 임상 및 병리학적 특성

Abstract
The marked gender influence on the occurrence of systemie lupus erythematosus (SLE) indicates that genetic and hormon@al factors would be important in the etiology of this illness. However, the clinical and pathologic features in male SLE patients are not well known. We studied 21 male lupus nephritis patients who had kidney biopsies between 1979 and 1993 at Seoul National University Hospital retrospectively. The male to female ratio was 1 to 8.6 during this period. Age at kidney biopsy ranged from 17 to 58 years (mean 30 years). Malar rash, oral ulcer, arthritis was less frequent clinical manifestations in male lups nephritis and hypertension, depressed C, level was more frequent clinical manifestation. The most frequent clinical presentaton at diagnosis was nephrotic syndrome(51%). The pathological findings in male lupus nephritis showed diffuse proliferative lesion (WHO Class IV ) in 13, membranous lesion (WHO Class V ) in 4 pa- tients and its proportion was not different from that of female lupus nephritis. But nephrotic syndrome was more frequent in male lupus nephritis with dif- fuse proliferative lesion than female diffuse prolifera- tive lupus nephritis. Of the 17 patients whose follow up period was over 3 months, 5 patients died snd 1 patient progressed to chronic renal failure. The actuarial survival rate of these 17 patients were 67% after 2 years and survival rate of male lupus nephritis was lower than that of female lupus nephritis.
