Abstract |
Renal disease with distinctive pathologic features was developed in a young woman with H mole. The renal abnormalities were manifested by severe pro- teinuria and microscopic hematuria and the blood pressure was elevated. Pathologic examination of the renal biopsy speci- mens showed distinctive glomerular abnormalities, characterized mainly by diffuse increase of mesan- gial matrix and proliferation of mesangial cells with some lobular accentuation. Capillary walls were dif- fusely thickened and lumens were largely obliterated. Trichrome stains revealed some subendothelial and messngial fuchsinophilic deposits. Ultrastructural study showed increased mesangial matrix and thick- ening of the capillary walls with mesangial interposi- tion and a few small subendothelial electron dense deposits. Overlying epithelial foot processes were par- tially obliterated. These findings were consistent with membranoproliferative glomerulonephritis type I. The renal abnormalities disappeared after suction- curratage of H mole. The clinical and pathologic features in this case suggest that the renal abnormalities were related to the uterine tumors and that the produetion of immune complexes and/or the activation of intravascular coagulation by the tumors may be the pathogenetic mechanisms. We report a case of H mole-associated membrano- proliferative glomerulonephritis. |
