소아에서 시행한 경피적 신생검 817예에 대한 분석

Abstract
To evaluate the distribution pattern of renal diseases based on needle biopsy, we analyzed 817 cases of renal biopsies at the Department of Pediatrics from Jul. 1978 to Jun. 1993. We excluded 17 cases of insufficient material and repeated biopsy in 70 patients. In l17 cases, the sex ratio (M: F) was 2.3: 1. The average age was 8.2 years in males and 8.5 years in females. Glomerular diseases were 98.5% of the total. The major glomerular diseases, in descending order of frequency, were; benign recurrent hematuria including thin glomerular basement membrane disease (BRH: 31.7%), minimal change nephrotic syndrome (MCNS: 25.0%), acute poststreptococcal glomerulonephritis (APSGN: 10.6%), Henoch-Schonlein purpura nephritis (HSPN: 9.9%), IgA nephropathy ggAN: 7.5%), mesangioproliferative glomerulonephritis (MsPGN: 3.8%), membranous nephropathy (MN: 3.1%). HBs antigenemia was found in 36 cases of which MN was the most frequent (68.2%). The causes of nephrotic syndrome were MCNS (63.7%), APSGN (7.5%), MN (6. 8%), HSPN (6.8%), FSGS (3.9%). The causes of hematuria were BRH (55.2%), APSGN (13.4%), HSPN (12.7%), IgAN (11.7%). A repeated biopsy was obtained from 70 patients because of evaluating the treatment response. Two cases of MCNS were confirmed by second biopsy to be FSGS. After the renal biopsy, complications were developed in 6 cases (0.7%). Blood transfusion was done in 5 cases, arteriovenous fistula developed in one case.
