말초혈액 단핵세포의 B2 - microglobulin 생산에 혈액투석막이 미치는 영향

Abstract
To evaluate the effect of different dialysis membranes on the production of B2-microglobulin (B2m) by peripheral blood mononuclear cells (PBMC), we measured the 2m concentrations in the supernatant of 24-hours cultured PBMC pre-and post-dialysis isolated from 9 chronic hemodialysis patients, who have used PMMA membrane over 6 months, using the following criteria: (1) in last dialysis with PMMA membranes (PMMA1) (2) in 6" times of dialysis with Cuprophan membranes (3) in further 6" times of dialysis with PMMA membranes (PMMA2). And we also investigated the effect of adding PMMA or Cuprophan membrane on the B2m concentra- tion in the supernatant of 24-hours cultured PBMC pre-PMMA1, pre-Cuprophan isolated from chronic hemodialyzed patients and normal control subjects. The results were as follows: 1) There were no significant differences in B2m con- centration of PBMC culture supernatant among normal controls, chronic renal failure patients and chronic hemodialysis patients. 2) There were no significant changes in B2m concentration of PBMC culture supernatant according to cross-over use of dialysis membrane as well as between pre-dialysis and post-dialysis. 3) In PMMA group, B2m concentration of PBMC culture supernatant with PMMA membrane particle (35. 2± 2.1 ng/ml) was low compared to those of PBMC with Cuprophan membrane particle (46.7+4.5 ng/ml: p<0.01) and spontaneous PBMC (54.9±3.7 ng/ml: p<0.001). 4) In Cuprophan group, B2m concentrations of PBMC culture supernatant with PMMA membrane particle (31. 1±1.4 ng/ml) and Cuprophan membrane particle (36.1± 2.2 ng/ml) were low compared to that of spontaneous PBMC (46.2±1.9ng/ml) (both p<0.001). 5) In normal group, B2m concentration of PBMC culture supernatant with PMMA membrane particle (34. 91.9 ng/ml) was low compared to those of PBMC with Cuprophan membrane particle (39.4±1.9 ng/ml: p<0.01) and spontaneous PBMC (44.0±3.6 ng/ml: p<0.01). These data suggest that PBMC from chronic hemodialysis patients with PMMA membrane behave a normal in vitro B2m production and the direct interaction between PBMC and dialysis membrane is unlikely to contribute to high plasma B2m concentration obser- ved in chronic hemodialysis patients. However, it seems that PMMA membrane has adsorptive capacity for B2m or inhibits the production of B2m by PBMC directly more than Cuprophan membrane.
