무증상성 요이상을 보인 환자에서의 임상상 및 병리학적 소견

Abstract
Asymptomatic urinary abnormalities(AUA) are familiar and vexing clinical problem. In a lot of previous studies, various clinical and pathologic results were reported. In order to compare with previous studies, we examined records of 50 patients with AUA who were taken renal biopsies between October 1992 and April 1995. 1) Twenty-four were male and 26 were female. The mean age was 35.42+1.81 years old. Twenty-t vo patients had isolated hematuria and 24 had hematuria with proteinuria and 4 had isolated proteinuria. Thirteen patients with hematuria(with or without proteinuria)had gross hernaturia. 2) In light microscopic findings, 9 cases(18%) had normal or minimal glomerular changes, 34(68%) had diffuse mesangial proliferative glomerulonephritis, 5(10%) had focal and segmental proliferative glomerulonephritis, and 2 had membranous nephropathy. Superimposed lesions were focal and segmental glomerulosclerosis(20 cases, 40%) and focal crescents(4 cases, 8%) 3) By immunoflorescence, there were glomerular deposits of immunoglobulin and/or C3 in 34 of the 41 biopsy specimens. IgA was observed in 33 cases, IgM in 13, IgG in 6, C3 in 27. Prominent IgA deposits were deteced in 30 cases, both prominent IgM and IgG in 2. 1solated rnesangial C3 depositions were observed in 5 cases. 4) By electron microscopy, there were mesangial and paramesangial electron-dense deposits in 32 of 40 studies. Thirty one cases had rnesangial deposits, and no dense deposits was obseved in 7. 5) Morphologic abnormalities did not correlate with the degree of proteinuria and urinary abnormalities(p>0.05). In conclusion, when AUA are present, it is more considerable to do renal biopsy, to evaluate the primary glomerular diseases, and to predict the prognosis of the diseases according the result
