만성 혈액 투석환자에서 투석간 체중 증가와 혈청 영양 인자와의 관계

Abstract
The study was undertaken to determine the effect of interdialytic weight gain(IDWG) on the bioche- mical indicies of nutrition in chronic hemodialysis patients in the assumption that increased weight gain might be associated with increased food intake. And we evaluated the difference between diabetics and non-diabetics. The diabetics (n=13) were older (60.0±13.2 vs 43.6±14.7, p=0.0008) and shorter (156.5±7.7 vs 162.2±7.6 (cm), p=0.024) than the nondiabetics. The serurn total protein (6.99±0.77 vs 7.62±0.41 (mg/dl), p=0.0107), serum albumin (3.98± 0.37 vs 4.43±0.31, p=0.0001), serum predialysis BUN (71.78±21.45 vs 86.02±16.64 (rng/dl), p=0.0178) and IDWG (1.92±0.82 vs 2.66±0.72 (kg), p=0.0035) and IDWG per dry weight (3.87±1.92 vs 5.17±1.62 (%), p=0.0221) were significantly lower in diabetics than nondiabetics. The dry weight, duration of hemo- dialysis treatment, residual renal function, KT/V and NPCR were not different. In all patients IDWG/dry weight was correlated with albumin (r=0.647, p= 0.0009) and predialysis BUN (r=0.506, p=0.0003). The age factor was also correlated with albumin (r=-0.556, p=0.0001) and predialysis BUN (r=-0.459, p=0.0012). Because the diabetic patients have lower serum albumin than nondiabetic patients in general and in this study, we evaluated the nutritional indicies separately by disease and described as below. Diabetics albumin predialysis BUN IDWG/dr R=0.542 y weight p=0.044 R=0.770 p=0.001 R=0.632 p=0.015 R=0.851 p=0.0001. Age R=-0.317 p>0.05 R=-0.369 p>0.05 Non-daibetics albumin predialysis BUN NPCR IDWG/dry weight R=-0.453 p=0.008 R=0.147 p>0.05 R=0.262 p>0.05 R=-0.359 p=0.039 R=0.737 p=0.0001 R=0.240 p>0.05 In conclusion, IDWG could be used as one of the nutritional indicies in chronic hemodialysis patients and the more increased interdialytic weight gain was associated with the better biochemical indicies of nutrition in diabetic hemodialysis patients.
