국소성 분절성 사구체 경화의 초기 형태학적 변화

Abstract
This study was conducted to identify early morpho- logical changes of focal segmental glomerulosclerosis (FSGS) induced by subtotal nephrectomy and repeat administration of puromycin aminonucleoside. Sprague Dawley rats were randomly divided into three groups. The Sham operation was performed in the one group(Sham, n=9), renal ablation was per- formed in the other group(Sham, n=9) and repeat intraperitoneal injection of puromycin aminonucl- eoside with 2 weeks of interval for 8 weeks in anoth- er group(PAN, n=9). The changes in blood pressure, serum creatinine and 24 hours urine protein were measured every other week during study and at 8 weeks after subto- tal nephrectomy and initial administrations of puro- mycin aminonucleoside. Histologic examination in rat glomerular lesions was performed. The blood pres- sure(mmHg) of RA(135.3±18.9) and PAN(125.6± 7.7) was significantly increased compared with Sham (118.9±22.0) at 8th week(p<0.01). Compared with Sham, RA and PAN showed signifieantly higher serum creatinine(mg/dL)(0.60±0.32 vs 0.79±0.17 and 0.72±0.12) at Sth week(p<0.05). Also, urine protein(mg/day)of RA(141.8±25.2) and PAN(137. 6± 38.9) was significantly increased compared with sham(15.1±4.3) at 8th week(p<0.01). The glomer- ular radius(pm) and area(10 mm) in RA(166.7± 22.9, 18.1 ±4.3) were increased significantly compaered with Sham(120.8±18.2, 8.7±1.5) and PAN(120.1±12.2, 8.9±1.3) Measngial proliferation and glomerular hypertrophy revealed in RA, but, podocyte injury without mesangial change nor glo- merular hypertropy in PAN. The results suggest that early histological changes by RA and PAN showed glomerular epithelial injury in PAN as a primary FSGS model and glomerular hypertropy in subtotal nephrectomy as a secondary FSGS model, respectively.
