범뇌하수체 기능 저하 및 중심성 뇨붕증을 합병한 출혈열 신증후군 1예

Abstract
Hemorrhagic fever with renal syndrome(HFRS) is an acute illness that most patients are recovered after its clinical course of febrile, hypotensive, oligu- ric, diuretic and convalescence phase but is known to result in rare sequale on recovery. There are a few reports on anterior pituitary dysfunction associated with central diabetes insipidus simultaneously as a sequale of HFRS. We experienced panhypopi- tuitarism and central diabetes insipidus developed in a 37-year-old male patient who had recovered from hemorrhagic fever with renal syndrome confirmed by high titers of antibody against Hantaan virus. Com- bined anterior pituitary function tests showed no response in all anterior pituitary hormones. Central diabetes insipidus was confirmed by water deprivation test and serum pitressin level. Sellar CT showed atropy in pituitary gland. In conclusion, prompt differential diagnosis, and proper management for the diabetes insipidus should be considered in patients who have unusal diuresis and long period of diuretic phase.
