B형 간염 바이러스로 인한 간경변 환자에서 육안적 신피질 석회화를 동반한 막증식성 사구체신염 1예

Abstract
Nephrocalcinosis is a condition characterized by the deposition of ealcium salts intracelluarly and extra- cellularly in kidney. The localization of the calcium deposition may vary, but it usually involves the me- dulla and corticomedullary area and occasionally the cortex. Cortical nephrocalcinosis is rarer than the medullary variety and usually the result of severe destructive cortical disease such as acute cortical necrosis, chronic glomerulonephritis or chronic pyelo- nephritis. The mechanism of nephrocalcinosis are not clear, but prolonged hypercalcemia and/or hyper- calciuria, excess circulating parathyroid hormone, impaired urinary acidification, impaired proximal tu- bular calcium reabsortion, and local injury to renal parenchyma are the factors that can predispose to nephrocalcinosis. We experienced a patient with macroscopic corti- cal nephrocalcinosis associated with membranoproli- ferative glomerulonephritis and liver cirrhosis due to hepatitis B virus. There have been no report of cortical nephrocalcinosis with chronic glomerulone- phritis in Korea, therefore we report this case with a review of some literatures.
