IgA신증의 예후에 영향을 끼치는 인자들

Abstract
A retrospective analysis of 36 patients with IgA nephropathy was carried out to assess the frequency of progressive and end stage renal disease and to identify predictors of progressive disease and also to evaluate the rate of progre- ssion of the disease in individual patients. The patients were followed for a mean period of 25. 3 months(1-87 months). Nine patients had impaired renal function at initial evaluation and an additional patient who had normal renal fun- ction at presentation developed renal failure during follow-up. Of these 10 patients 8 had progressive disease and 5 of these 8 patients progressed to end-stage renal disease and dialysis. Elevated serum creatinine(Scr l. 8mg.) and high blood pressure(160/95 mmHg) at initial evaluation appeared to be important predictive indices of progressive disease. Mean doubling time for Scr was 8. 8 months and mean time required for 10% reduction of reciprocal of Scr was 5.4months. The rate of progression is probably determined by the num- ber of risk factors that each individual may have.
