낭창성 신염 환자에서 신손상 정도에 따른 혈청 전해질 및 산염기 평형에 관한 연구

Abstract
We studied serum electrolytes and acid-base composi- tion in 74 patients with lupus nephritis (LN) on the graded degrees of renal failure, compared to 65 patients with the other chronic renal disease (Non-LN). LN patients were selected as cases showing microscopic hematuria or more than 300 mg of 24 hour urinary protein excretion among patients with systemic lupus erythematosus diagnosed by clinical criteria for S.L.E. of American Rheumatism Association without evidence of S.L.E. and according to serum creatinine concentra- tion, all subjects were divided into following 3 groups: group 1; Cr<1.5mg/dl, group 2; 1.5<=Cr<3.0mg/dl, group 3; 3.0<=Cr<6.0 mg/dl. The patients who received medications or had a disease influencing the electrolytes and acid-base balance were excluded. The results obtained were summarzed as follows: 1. The mean age and male to female ratio of LN patients were 30.9 years and 1:10.5 in group 1, 30.2 years and 1:5.7 in group 2, and 31.6 years and 1:4 in group 3, respectively. So, the majority of LN patients were between 20 and 40 years with female prepondance. 2. Compared to Non- LN patients, the serum potas- sium concentrations tended to be higher in LN patients of all 3 groups and in group 3, statistically significant difference were seen (p<0.05, 5.8±0 9mEq/L vs 4 8±0 8 mEq/L) 3 The total CO, contents in LN patients showed loweer values than Non LN patients in all 3 groups and the difference were significant in group 2, as 18. 4±3 2mEq/L vs 23.2±3.4mEq/L(p<0.05) 4 The serum sodium, chloride concentrations and the anion gaps of LN patients in each group. From the above results, it seems that the degree of acidosis and hyperkalemia compared to the other chronic renal disease patients Considering that tubulointerstitial nephropathy was somewhat frequently disecovered in lupus nephritis, our findings were suspected as part of renal tubular dysfunction in these patients
