Glycyrrhizince acid에 의해 유발된 Mineralocorticoid Excess Syndrome 1례

Abstract
Glycyrrhizinic acid is an extract of licorice which is known to have mineralocorticoid effect by inhi- biting lll3-hydroxysteroid dehydrogenase and allo- wing endogenous cortisol to bind mineralocorticoid receptor in kidney. We report a 59-year old female patient who had ingested 150mg of glycyrrhizinic acid daily for 3 months. She was found to have hypertension, hy- pokalemia(K: 1.8mEq/L), metabolic alkalosis and suppressed renin-aldosterone level. After stopping ingestion of glycyrrhizinic acid, her serum potassium level increased spontaneously in 5 days, but low renin-aldosterone level was persisted until discharge from hospital. We conclude that history of licorice(glycyrrhizinic acid) ingestion should be obtained in patients ma- nifesting hypokalemia with metabolic alkalosis and hypertension.
